New User Registration - Select "Consumer" if you're the client or "Provider" if you're the aide/attendant:


Step 1: Select your State, Program and Role

Once you've completed this step, you will click the NEXT button and it will ask you questions about your personal account, then you will click the NEXT button again which will allow you to create your login and password.

FOR PAPER TIMESHEETS:

 

Contact PPL and request that they mail you some, login to your ppl account and click on Consumer Forms or ask your "Home Visit Rep. if he/she can bring you one to make copies from when she makes the next monthly visit.

 

It is recommended that you make copies of the paper timesheet before you get down to your last.
