(Even though there are a number of waivers, the Consumer-Directed Service is a Component of only the following Waivers):

 

- Elderly and Disabled with Consumer Directed (EDCD);

 

- Intellectual Disability Waiver (ID); 

 

- Developmental Disability Waiver (DD); and

 

- Early Periodic Screening Diagnosis and Treatment ("EPSDT").

Commonwealth Coordinated Care ("CCC")

Commonwealth Coordinated Care is a program that allows persons on Medicare to participate in the Consumer Directed Services program along with Medicaid.

 

Medicare Insurance Providers who participate in the program are:

 

 

- Anthem

- Humana

- VA Premier

Comfort Facilitators is seeking an addition to our family of "HOME VISIT REPS./SF"...

 

             POSITION DESCRIPTION:

  • "This is an Independent Contractor" position;
  • Individual must be a responsible and compassionate person to visit elderly and/or disabled persons on a   monthly basis;
  • Individual must be willing to visit approximately 20-30 clients a month;
  • Individual can work at their own schedule (i.e., mornings, evenings and/or weekends), provided that the        client has agreed to that date and time;
  • Individual responsibility will be to make sure that the client's information on pre-typed Medicaid/Medicare      forms are accurate and current at all times; and
  • Individual must obtain the signature of the client or the responsible party during each monthly visit.

    QUALIFICATIONS:
  • Must have own, reliable transportation and valid Driver's License;
  • Must have access to a computer and internet;
  • Must sign an Independent Contractor's Agreement and accompanying documents (i.e. application, I9, etc.);
  • Must pass a criminal background / child protective services background check that must be performed               by the Virginia State Police Department (Medicaid/Medicare Requirement). It is your responsibility to pay           for a Criminal Background Check and DSS Children Protective Service Check, totaling $30;
  • Must read and write "English";
  • Must be able to scan and email all visit sheets and payment logs for the week every Thursday (in order to be   paid on Monday);
  • Preferred background in Nursing (i.e., CNA, RN, LPN, etc.) and
  • Must meet with the Owner every Monday.
